You may adapt these templates for use in your own organization. Every template comes in two formats. Word is the one to open if you will edit the document, track changes, and circulate it for approval. Markdown suits teams that keep policy in Git or a wiki next to their configuration.

The IT Change Management Policy is a complete, ready-to-adapt policy covering change categories, risk classification, approval roles, and the six-monthly firewall review. Treat every value in it as a default to tailor. The Change Request Template is the short, per-change record that sits under it, and the Rule Documentation Form captures what an auditor or the next administrator needs to know about each rule. The zone matrix also comes as CSV, so you can open it in a spreadsheet and extend it.
